| In April 2006, the Health and
Social Care Department changed the way it
provides social care services for adults. This
booklet explains some of these changes, which
have been made to improve the quality of the
services the department provides. The booklet
also explains how you and your family can get
the help you need. |

| You are entitled to
a carer’s assessment if you undertake a
substantial and regular caring role for another
person. A carer is generally defined as a person
of any age who provides unpaid help and support
to a relative, friend or neighbour who cannot
manage to live independently without the carer’s
help, due to frailty, illness, disability or
addiction. |

The term Direct
Payment is commonly used when referring to
Self-Directed Support. Guidance from the
Scottish Executive published in July 2007 uses
the term Self-Directed Support as it encompasses
the potential for its usage more fully than
Direct Payment. |

risk? |

| Most people with mental
illness, physical or learning disabilities, or
other care needs live their lives comfortably
and securely, either independently or with
assistance. However, for a small number,
dependence on others may put them at risk and
produce conflict, exploitation, abuse or
neglect. |

Service |

| The service is one
of a range of services providing care in the
community to help you, your relatives and
carers.This leaflet provides information for
people needing help to stay in their own home.
Each person’s needs are different and so
individual arrangements vary widely. The help
you need and would like to have will be
discussed fully with you. If you are at home
someone will be able to assess your needs and
can provide further information about the
service. If you are about to leave hospital your
care plan will include this
assessment. |
